Medical Plan
SummaCare is offering a new medical plan for 2025. Our plan is a PPO plan that offers both network and non-network coverage nationwide.
Plan Highlights

- $500 per individual or $1,000 per family
- $0 Preventive Health
- $10 Office Copay with primary care physician
- $20 Office Copay with a specialist
- $30 Copay at Urgent Care
- $150 Copay for Emergency Room
Employees who enroll in the SummaCare health plan will receive an employer paid incentive of $200 single/$400 family which is disbursed at the end of every quarter ($50 single/$100 family). HRA funds will roll over annually and can be spent at any time regardless of the year earned.

Utilization Management Programs include step therapy, quantity limits and prior authorization. The formulary will indicate if these programs apply.
MedImpactSave™ is a program where the pharmacy will compare the member’s copay to any available discounts and the retail price and charge the lowest available price for Tier 1 generic prescriptions.
Specialty MedImpact Assis™: (Copay Assistance Program for Specialty drugs) Specialty drugs are required to be filled at AcariaHealth. AcariaHealth and MedImpact will work behind the scenes to maximize available manufacturer-funded copay assistance programs.
Opiod Drug Utilization Review (DUR) Program ensures appropriate utilization and medical necessity of opioid analgesics prescribed.
